Service Above Self Nomination Form

 

     This year we will be awarding Service Above Self Awards in two categories:  individual honorees and a community impact honoree.

     Individual Service Above Self nomination forms are immediately  available on the Rockford Rotary website:  www.rockfordrotary.org.  Click on Membership and you will find the nomination form for individuals to the left.  The forms will also be available at meetings in the near future.

     Community Impact nomination forms will be available very soon.
